The best way to attach it is by laying the front flap of the pouch on your bra, lacing the ribbon under your strap, and attaching the Velcro. Then you can tuck the pouch in wherever comfortable, although under the breast seems to be the most comfortable spot. The ribbon is adjustable, and you can extend it depending on what you are wearing and where you want to place the pouch.  Place the Velcro side against the bra.
